PHYSICAL EXAMINATIONS ARE NEEDED FOR:

1. Pre-kindergarten (children entering school for the first time).
2. New students
3. Kindergarten

N.J. MANDATED HEALTH SCREENINGS:

N.J.A.C. 6A:16-2.2 (k) details when these screening procedures must occur. The results are recorded on the student's school health record, A-45. 
Parents/guardians are notified of any results that need further evaluation.

• Vision screening: Grades K, 2, 4, and 6

• Color Vision: Grade 1

• Hearing Screening: Grades K, 1, 2, 3

• Scoliosis: Grade 5

• Height and Weight: All Grades

• Blood Pressure: All Grades

• Testing for tuberculosis: P.P.D. (Purified Protein Derivative) - Mantoux test – only students transferring from locations for which the state requires testing.

IMMUNIZATIONS

• Diphtheria - A minimum of 4 doses of diphtheria and tetanus toxoid and pertussis vaccine (DPT) one dose of which is given on or after the 4th birthday .Any 5 immunizations will meet the requirement.

• Poliomyelitis - a completed series consisting of three (3) doses of live, trivalent, oral poliovirus vaccine (OPV), one dose of which is given on or after the 4th birthday. Any 4 immunization will meet the requirement.

• Measles, Mumps and Rubella – One dose given on or after the 1^st birthday,

• Measles – A second dose of measles vaccine is required before entrance into kindergarten.

• Hepatitis B – 3 doses must be given before entrance into Grade K or 1. If the child is not immunized before school starts, dose 2 must be given no later than 3 months after dose 1. Dose 3 must be given within 12 months of dose 1.

• Varicella – Must be given on or after the 1^st birthday.

•  Dtap & Meningococcal Vaccines - Required for all 6th Grade Students, due by September 30.

• Provisional Admission: If a student does not meet the above requirements, a physician's statement must be presented that the immunizations will be given in a reasonable length of time consistent with the State of New Jersey Chapter 14 Guidelines.

MEDICATION

If medication must be given during the school day, we will cooperate with parents as best we can. Only prescription medication brought in the pharmacy bottle for the particular child with a current date can be given by the school nurse. If she is not available to give the medication, the parent will have to make other arrangements. A note from the parent must accompany the prescription medication requesting that it be given, with MD form completed. Inhalers may be self-administered with the physician's approval.

Over-the-counter medications can only be given in school if they are accompanied by the physician's note and a note from the parent.

Throat lozenges may be brought to school only if absolutely necessary. They must be accompanied by a note from the parent. The teacher will hold them and allow the child to have as needed.

ACCIDENT - FIRST AID

• The school makes every attempt to provide an environment in which the child will be safe from accidents. If an accident or sudden illness occurs, First Aid will be administered and the child's parents will be requested to come for the child. Transportation cannot be provided.

• No care beyond First Aid, defined as the immediate and temporary care in case of accident or sudden illness, will be given by the school physician or nurse.

All accidents must be reported to the school nurse.

COMMUNICABLE OR NUISANCE DISEASES

Any child or person suspected to be ill with a communicable disease shall be excluded until a statement is presented that such child or person is not likely to transmit infection.  A child will be excluded if he/she has a fever of 100 degrees, is vomiting or has diarrhea. Other communicable conditions, so-called “nuisance” diseases, also require exclusion. They include head lice and/or nits, ringworm, scabies, impetigo, pinworm, conjunctivitis (pink eye), and open skin lesions.

NJ FAMILY CARE INFORMATION - Mandated Health Coverage for all Students age 18 and under

Recently signed legislation now mandates health insurance for all children age 18 and under. The requirement can be met through (1) health insurance provided by your employer, (2) a health insurance policy you purchase directly, or (3) through NJ FamilyCare, which provides free or low cost insurance for children and their parents. Enrollment in school insurance programs ONLY does NOT satisfy this mandate.

Information on NJ FamilyCare and eligibility is available through the school nurse in each of our school buildings. Uninsured families may contact the school nurse to learn more about the application process for NJ Family Care. All information will be kept strictly confidential.
